Migraines? You Can Still Fly.

Migraines and chronic headaches are now a CACI condition — one of the fastest pathways to certification. With the right documentation, your AME can issue your medical certificate at your appointment.

The FAA Pathway

Migraines go through CACI (Conditions AMEs Can Issue), which means your Aviation Medical Examiner can approve your certificate on the spot — no waiting for Oklahoma City. The process typically takes 1 to 2 weeks.

The FAA requires that your migraines are controlled, you're on acceptable medications, and there are no neurological symptoms that would impair flight safety.

Medication Wait Times Reduced

As of August 2025, the FAA reduced migraine medication wait times to as little as 24 hours. This is a major improvement for pilots who rely on medication to manage their migraines.

Bring a clinical progress note from your treating physician (within 90 days) and your current medication list to your AME appointment.

Migraines with Aura

Standard migraines with typical aura are generally certifiable through CACI. However, migraines with prolonged neurological symptoms may require additional review, including a neurology evaluation.

Complex migraine presentations don't automatically disqualify you — they just need more documentation to demonstrate safety.
